Symptom & Medication Support

Support for patients managing treatment side effects at home and needing help knowing what is expected, what needs attention, and when to contact the oncology team.

Included in monthly support packages:
Symptom check-ins, medication organization, side-effect education, treatment-cycle preparation, tracking concerns, and escalation guidance.

Optional add-ons may include extra calls, urgent support appointments, medication review preparation, or in-home assessment where available.

Education, Advocacy & Navigation

Support for diagnosis, treatment decisions, appointment preparation, scans, or changes in the care plan.

Included in initial consult and care planning:
Clarifying needs, identifying priorities, organizing questions, preparing for appointments, and developing a personalized support plan.

Ongoing packages may include regular check-ins, care coordination support, advocacy preparation, and family/caregiver guidance.

Nutrition, Movement & Whole-Person Care

Support for appetite, hydration, energy, routines, independence, coping, and caregiver needs during cancer care.

Available across support packages:
Nutrition and hydration strategies, gentle movement planning, fatigue management, energy conservation, emotional support, and caregiver guidance.

Optional add-ons may include focused planning sessions, caregiver meetings, or home/hospital visits where available.

Digital Oncology Support

Support for using patient portals, symptom questionnaires, treatment calendars, digital notes, or remote monitoring tools.

Helpful for monthly support members:
Assistance with digital symptom reporting, portals, care notes, and knowing when symptoms need direct follow-up.

This support helps bridge technology and clinical care between oncology appointments.

Packages may include a set number of monthly support calls.
Additional calls, in-home visits, hospital visits, ER support, and travel-related costs may be available as optional add-ons.
