After-Hours Symptom Coaching
Practical, evidence-informed support for day-to-day symptoms and treatment side effects. Together, we map what’s changing, choose simple self-management steps, identify red flags, and plan how best to communicate your concern to your care team—rooted in whole-person care inspired by Medicine of the Person.
Drawing on over two decades in oncology nursing and years of experience providing virtual symptom support through both phone-based and digital health platforms, your coach understands how to recognize changes early, translate complex guidance into plain language, and help patients feel supported even outside regular clinic hours.
Proactive Symptom Check-Ins (15 min)
Short, focused touchpoints to stay ahead of symptoms during treatment changes or after an ER/urgent care visit. If this sounds helpful, please ask your coach about adding a proactive check-in to your plan.
How It Works
1. Book your session
Choose a 15-, 30-, or 45-minute slot—available virtually (video or phone) or in-person by request.
2. Share concerns
Complete a brief pre-session form or note your main symptoms when booking so we are ready to help immediately.
3. Meet your coach
Together we review what’s happening, identify early warning signs, and outline what can be managed at home vs. reported to oncology.
4. Receive summary
Optional—receive a brief summary of key points, self-management strategies, and follow-up reminders.
Spiritual Support (by invitation):
If desired, sessions may include a brief reflective moment, mindfulness pause, or prayer. This is always optional, inclusive, and guided by your comfort and beliefs.
